This list includes all crashed, destroyed, shot down, and scraped airframes that are no longer in service with the United States Air Force or Army. This list includes two airframes that never actually flew - 59-04982 and airframe build number 6 (no tail number was ever assigned). Included additionally are the four surviving A models now resting peacefully in museums (60-03451, 61-02408, 64-13149, 65-07992), as well as 59-04986, the A model once utilized as a sheet metal repair training device at the Structural Repair School located at Fort Eustis, Virginia. From Andy Dulay: My CH-47 crash in Vietnam (66-19032). This photo also showed up in an issue of Look magazine, circa 1968. The cause of the crash was a dual hydraulic failure at low level and high speed. We were inverted and traveling backward at about 50 knots by the time we hit the ground. Amazingly, nobody was killed, but everyone in the crew was injured. Photographer unknown. If anyone has more info about this crash and/or the tail number, please email us.
